Registration

There are 4 ways to register:

• 1. Online: Available only for Three Day Symposium registration. Register online (credit card only via secure website)
• 2. Fax: 715-839-8680 (credit card or entire purchase order hard copy); fax the completed registration form and entire purchase order hard copy
• 3. Phone: 800-839-4584 or 715-839-8055 credit card. Call Customer Service Department, 8:00 a.m. – 5:00 p.m. CST
• 4. Mail: Check, money order payable to HEALTH ED, credit card information, or entire purchase order hard copy to:

Acceptable forms of payment include:

• 1. Credit Card: Visa, MasterCard, Discover, American Express
• 2. Check or Money Order: payable to HEALTH ED.
• 3. Purchase Order: must include a copy of the original purchase order with the following information - PO number, company name & address, and any other billing address.

Registration Policy

A completed registration form, payment, or entire purchase order hard copy is required at time of registration for each attendee. Use only one registration form per person. Please copy the registration form for multiple registrations. Pre-Registration rates apply only when registering with payment in full before noon CST on November 10, 2010.

*Single day rates are not available after noon CST November 10, 2010 deadline. Single day registration is available only by fax, mail, or phone. Day attending must be specified with registration.

Please note:
Space permitting - if registering for the Three Day Symposium after noon CST, November 10, 2010 fees for Professionals will increase to $450 and fees for Family/Caregivers will increase to $300.

Prior to the Symposium you will be mailed a Welcome Letter listing important onsite information including the URL to download your Symposium breakout session eHandouts.

Receipt/Confirmation of Registration:
If registering online you will automatically receive an email confirmation, otherwise email your confirmation request to smartin@health-ed.com.

Cancellations, Deadlines/Fees, Substitutions, and Refunds

To cancel your registration, please call our Customer Service Department at 1-800-839-4584. A refund, minus an administrative fee, is available for cancellations if received no later than the deadline dates listed below. No refunds will be granted after October 28, 2010. Purchase orders are considered the same as a check or credit card and the same policy will apply. Please email smartin@health-ed.com at least 3 days prior to the program for substitutions.

Cancellation Deadlines

Three Day Registration:

• Now through September 10, 2010 - $25 cancellation fee
• September 11 – October 10, 2010 - $100 cancellation fee
• October 11 – October 28, 2010 - $200 cancellation fee
• After October 28, 2010 - No refund

Single Day Registration:

• Now through October 28, 2010 - 50% of the daily tuition
• After October 28, 2010 - No refund

By registering for the Symposium, you understand and agree that any photographs, videos or audio recordings taken of attendees during the course of the event, as well as written feedback, may be used to promote future HEALTH EDucation Network, LLC events, seminars, or HEALTH ED itself at our discretion.
