Internet: On each program page there is a Register Now button below the date and location. Click on it, you will be entering our secure web site, you have a choice of registering with a credit card payment or fill out the form to print and fax or mail it to HEALTH ED.

Group Registrations: HEALTH ED encourages group registrations. You may do this by clicking on Register Now, please select the "Print to order by fax or postal mail" option under "Ways to Order" fill out the form and submit with payment or simply call to register groups at 800-839-4584. Currently, you can not register for groups online. If you have questions about our online registration please contact our website manager at 866.367.5697 ext 2019.

Fax: 715.839.8680 (credit card or purchase order). A registration form is available online, click on the Register Now button and choose the "Print to order by fax or postal mail" option. Fill out the form completely, then click continue, print out the form and finish the payment method information.
